Request An Exam

Please use the following form to request an evaluation from Proficient Medical Evaluators.  This form may be used for a Panel Qualified Medical Evaluation (PQME), Qualified Medical Evaluation (QME) Independent Medical Evaluation (IME) or an Agreed Medical Evaluation (AME), within the state of California.  Please see a map of our locations.

How to schedule an Examination:

We invite you to first view our medical examiners. When you are ready to make an appointment, Request An Exam, and upload the information as prompted to start the scheduling process or give us a call at (877) 234-9556.

    Upload your demographic sheet and Panel Letter and submit here. Once submitted you will receive a response from a PME staff member within 24 hours with a time and date to schedule an Evaluation.
